A Brainstem Auditory Evoked Response test, often called a BAER, Auditory Brainstem Response (ABR), or Brainstem Evoked Response Audiometry (BERA), is a non-invasive and specialized diagnostic tool to test the hearing pathways and the nerves from the inner ear to the brainstem and to the brain’s auditory centers.
How Does a BAER Test Work?
Describing a hearing test is fascinating. Sound energy travels through the outer ear and the middle ear to the inner ear and it is transduced to electrical signals. The BAER test captures the brain’s response to the electrical signal. The auditory nerve transmits sound information through several stages in the brainstem to the auditory cortex of the brain. This is where the brain becomes aware of sound.
Why Is a BAER Test Done?
A BAER test is done to evaluate the auditory pathway and brainstem pathway disruption when hearing loss cannot be tested. Typical indications include:
- Newborn hearing screening – BAER is used to test for hearing loss in newborns upon their hospital discharge. BAER is also used for confirmatory testing when a newborn fails an initial hearing screening.
- Hearing assessment in infants and young children – Many behavioral hearing tests require the child to actively engage in testing. If the child happens to be of a younger age, or is developmentally unable to take part in the hearing test, BAER can be used.
- Suspected acoustic neuroma (vestibular schwannoma) – Acoustic Neuroma, also referred to as Vestibular Schwannoma, is a benign growth typically of a slow nature. These neoplasms can affect hearing and create one-sided hearing loss, along with tinnitus.

- Tinnitus evaluation -In the presence of tinnitus, BAER can be used to analyze the auditory neural pathways to the brainstem.
- Intraoperative monitoring – BAER is used during certain neurosurgical and skull base procedures to monitor the auditory nerve in real time and reduce the risk of hearing damage
- Monitoring in ICU patients – BAER can assess brainstem function in comatose or critically ill patients who cannot cooperate with clinical testing
- Assessment of brainstem function – As part of a broader neurological evaluation when brainstem involvement is suspected
How to Prepare for a BAER Test
Preparation for a BAER test is minimal. The following steps will help ensure the best possible recording quality:
- Wash your hair the night before or on the morning of the test. Do not apply any conditioner, hair serum, gel, spray, or styling products, as these create a barrier between the electrodes and the scalp and can reduce the quality of the recording.
- Continue taking all regular medications unless your neurologist has specifically advised otherwise. Do not stop any medications without medical guidance.
- Avoid caffeine for at least two to three hours before the test, as it can increase alertness and make it harder to remain still and relaxed during the recording.
- Eat normally before the test. There is no requirement to fast.
- For infants and young children, the neurologist may recommend mild sedation to help the child remain still and relaxed throughout the procedure. If sedation is planned, specific fasting instructions will be provided in advance, and you will need to arrange for someone to drive you and your child home after the test.
- Dress comfortably in loose-fitting clothing. No special preparation of the ears or hearing aids is required in advance, though hearing aids will be removed during the test itself.
What to Expect During a BAER Test
Before the Test
On arrival, your neurologist will review your symptoms and clinical history and explain the procedure in full. If the test is being performed on a child, the process will be explained to the parent or guardian and any concerns will be addressed before the test begins.
During the Test
You will be asked to lie down or sit in a comfortable, reclined position. Small electrodes are attached to specific points on your scalp – typically at the top of the head and on each earlobe or mastoid bone (the bony prominence behind the ear) – using a conductive gel or adhesive paste. The electrodes do not deliver any electrical stimulation; they only record your brain’s naturally occurring responses.
Earphones are then placed in or over the ears. A series of brief, repetitive clicking sounds are played through the earphones, one ear at a time, at varying volumes and frequencies. You are not required to respond to the sounds in any way. You simply need to remain as still and relaxed as possible – or, in the case of a sleeping infant, remain asleep.
After the Test
Once the recording is complete, the electrodes are removed and any gel is cleaned from the scalp. There are no restrictions following a BAER test. If no sedation was used, you can return to your normal daily activities immediately – including driving and work.
If sedation was administered – most commonly in young children – you will need to remain in the clinic for a short observation period before discharge. Arrange for someone to drive you and your child home, and allow time for the sedation to wear off fully before resuming normal activities.
Risks and Safety
The BAER test is a very safe, painless, and non-invasive procedure. The electrodes do not deliver any electrical current into the body – they only record the brain’s naturally occurring electrical activity in response to sound. There is no radiation involved.
There are no known risks or side effects associated with the BAER test itself. The clicking sounds used are carefully calibrated and are not harmful to hearing.
If sedation is used for a young child or an uncooperative patient, the sedative agent is chosen carefully based on the patient’s age and medical history, and the child is monitored throughout the procedure by trained clinical staff.
Results
Following the test, the neurologist analyses the recorded waveforms in detail. The five characteristic peaks of the BAER waveform are examined for their absolute latencies (the time from the sound stimulus to the appearance of each wave), inter-peak latencies (the time between specific waves), and amplitudes (the height of each wave).
Findings may indicate:
- Normal auditory brainstem function – All five waves are present at the expected latencies and amplitudes, indicating intact conduction from the auditory nerve through the brainstem
- Peripheral hearing loss – Changes in the early waves (Wave I) that suggest a problem at the level of the cochlea or auditory nerve, rather than the brainstem
- Retrocochlear pathology – Prolonged inter-peak latencies suggesting disruption beyond the cochlea, as seen in acoustic neuroma or auditory neuropathy
- Brainstem demyelination – Prolonged latencies or absent waves indicating disruption of myelin in the brainstem auditory pathways, as seen in multiple sclerosis
- Brainstem structural lesions – Asymmetric or absent waveforms suggesting a focal lesion such as a stroke, tumour, or inflammatory condition
- Absent or severely abnormal responses – Indicating profound hearing loss or significant brainstem dysfunction
